Athletic Paperwork
To register your student to play sports, families will need to login to their Parent Portal account, and click on the “school store”.
Click on the specific sport(s) and school your student will register/play for. For more detailed information please see below.
List of things to make sure you have completed before your student(s) are able to participate:
- Fill out Online Athletic Paperwork – Both Parent and Student must log in under their Infinite Campus log in and electronically sign the sports paperwork.
Parent Infinite Campus: Username: Email Address Password: (can click on “Forgot Password”)
Once you have signed your portion – Under “School Store” please click on “MY CART” and click “SUBMIT” once this is done, have your child log in to their Student Infinite Campus for their portion. (You will receive a confirmation e-mail once your paperwork has gone through.)
Student Infinite Campus – Username: Student ID Number Password: Password student uses to log in to their Chromebook
Click “Documents” – List of sports documents will be listed, have them click on each one and scroll until they can click sign document, submit them and form shows “complete”.
- Turn in Athletic Physical OR Alternate Physical Form (paper or electronically) – Cannot participate without this!
- Turn in $50 Athletic Fee to the School before 1st game (Cash or check made out to Kenosha Unified). All athletic fees will be applied after rosters are finalized.
Physical form and Alternate Physical form can be found above by clicking on “Click here for athletic forms”.
If your student athlete HAS NOT received an athletic physical in the last calendar year (valid physicals are those after 4/1/2020) they must try and get one completed ASAP and return the first Physical form to us (2). Kenosha Urgicare can provide athletic physicals for $40.
If your student athlete HAS had an athletic physical and you have previously submitted a doctor signed form to the main office in the last calendar year (valid physicals are those done after 4/1/2020) then you just need to fill out the Alternate Physical Form either electronically or via paper form.
Physicals or Alternate Physicals can be faxed to 262-359-2184, emailed to rperry@kusd.edu or dropped off to the main office Monday-Friday 8-3:30pm.
If you have any questions please feel free to call the office at 262-359-2240. Please make sure and get these done ASAP!! Players CAN NOT practice unless all paperwork is in!!!
