Athletic Event Announcement:

Students are required to wear their grade level school photo IDs to all athletic events, both home and away. Also, students that have received an Office Disciplinary Referral (ODR) during the week of or the week prior to the athletic event, are not eligible to attend.

Physical Examination Card (dated on or after April 1, 2017) - En Espanol

Walk-In Physicals ($30) are available at Kenosha UrgiCare.

Alternate Year Physical Card for students with a physical after April 1, 2017 - En Espanol

Concussion Information Form (every sport played)

Risk of Injury Form - Personal Insurance Provider must be provided (every sport played)

Emergency Form (every sport played) - En Espanol

Activities Code of Conduct (every sport played)

Athletic Code of Conduct - Amendments (every sport played)

Athletes must have all of the above forms completed and turned in before they're allowed to participate in any tryouts or practices. Only one physical or alternate physical card and Concussion Form are needed for the entire school year. However, all other forms must be completed for every sport played.

Athletic Fee:

$50 Middle School Fee ($100 student cap, $200 family cap)

Softball, Girls September and October 6th, 7th, 8th

Wrestling November and December 6th, 7th, 8th

*6th Grade students must weight more than 100lbs. to be eligible to play football.
