CEP allows eligible schools to serve healthy breakfast and lunch meals to students at no charge each day.<\/span><\/p>\n<p><span style=\"font-weight: 400\">LakeView K-8 Academy is a non-CEP school. However, students attending non-CEP schools may still qualify for free or reduced-price meals based on household income.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">All families are encouraged to<\/span><a href=\"https:\/\/www.myschoolapps.com\/\"> <span style=\"font-weight: 400\">complete the annual Free and Reduced Meal Application<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Completing this form may help families and schools qualify for additional benefits, including:<\/span><\/p>\n<ul>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Free or reduced-price meals at non-CEP schools<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Title I funding support, which helps schools provide additional academic services, instructional support, and resources for students<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Wisconsin School Day Milk (Pre-k\/Kindergarten snack milk)<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Athletics fee waivers<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Fine arts fee waivers<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Advanced Placement (AP) testing fee waivers+<\/span><\/li>\n<\/ul>\n<p><b>Meal Prepayment System<\/b><\/p>\n<p><span style=\"font-weight: 400\">Mosaic is a computerized meal tracking system that allows parents to make prepayments into their student\u2019s meal account. This system eliminates the problems associated with students carrying money for lunch everyday. Parents may make prepayments into their student\u2019s accounts by the week, month or year. <\/span><span style=\"font-weight: 400\">Money may be added to your student\u2019s account by filling out a prepayment envelope and returning it to the school or the Food Service department in the ESC building.\u00a0 In person credit card payments can also be made at the ESC. <\/span><span style=\"font-weight: 400\">Money may also be added to your student\u2019s meal account by using<\/span> <a href=\"http:\/\/myschoolbucks.com\/\"><span style=\"font-weight: 400\">MySchoolBucks.com<\/span><\/a><span style=\"font-weight: 400\">.<\/span><\/p>\n<p><b>Resources and Information<\/b><\/p>\n<ul>\n<li style=\"font-weight: 400\"><a href=\"https:\/\/www.kusd.edu\/district\/departments\/food-service\/menu-pricing\/\"><span style=\"font-weight: 400\">Menu Pricing<\/span><\/a><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Breakfast and lunch menus can be viewed daily on <\/span><a href=\"https:\/\/schools.mealviewer.com\/school\/LakeviewK-8Academy\"><span style=\"font-weight: 400\">Meal Viewer<\/span><\/a><span style=\"font-weight: 400\"> through the KUSD website.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Breakfast service is from 7:30 a.m. until 8 a.m.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><a href=\"https:\/\/www.kusd.edu\/district\/departments\/food-service\/school-meal-charge-procedures\/\"><span style=\"font-weight: 400\">School Meal Charge Procedures<\/span><\/a><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Our 4K program participates in the snack milk program for which there is a fee of <\/span><span style=\"font-weight: 400\">$44 yearly<\/span><span style=\"font-weight: 400\">.\u00a0<\/span><\/li>\n<li style=\"font-weight: 400\"><span style=\"font-weight: 400\">Parents can <\/span><a href=\"https:\/\/forms.gle\/dRYdxg2z9FqSRwCp8\"><span style=\"font-weight: 400\">order birthday or celebration treats<\/span><\/a><span style=\"font-weight: 400\"> from our kitchen to be delivered to the student. Prescription medications require both parent\/guardian AND health care provider signature.<\/span><\/p>\n<ul>\n<li><a href=\"https:\/\/www.kusd.edu\/wp-content\/uploads\/2017\/07\/medication-authorization.pdf\">Medication Authorization Form \u2013 English (PDF)<\/a><\/li>\n<li><a href=\"https:\/\/www.kusd.edu\/wp-content\/uploads\/2017\/07\/medication-authorization-sp.pdf\">Medication Authorization Form \u2013 Spanish (PDF)<\/a><\/li>\n<\/ul>\n<p><span style=\"font-weight: 400\">Medications that are ordered daily for the morning are to be given at home.\u00a0\u00a0\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400\">All medications must be stored in the health office with the exception of EPI-PENs, inhalers, and certain diabetic supplies. These medications may be carried IF the health care provider indicates this on the medication authorization form.<\/span><\/p>\n<p><span style=\"font-weight: 400\">When providing medications to the office, they must be in their original, unopened, containers and not be expired.<\/span><\/p>\n<p><span style=\"font-weight: 400\">Please be sure that your students are up-to-date on their immunizations.
