Form W2: Wage and Tax Statement
Form W2 is scheduled to be both mailed to your home address on record with KUSD and posted to Employee Online by Friday, Jan. 31. Please remember that you can verify your home address via Employee Online.
**New on the W2 this year you will find HSA contributions in box 12b with Code W. This amount is a combination of both Employer and Employee contributions.
**If you have a 403(b) or a ROTH account, you will be receiving a supplemental Form W2 with that additional information. Please note the codes that correspond to the values in each of those fields. If you have any of these supplemental items you will receive two tax documents. These will be mailed on the same day but are two separate documents.

Form 1095C: Employer-Provided Health Insurance Offer and Coverage
Form 1095C is scheduled to be both mailed and posted to Employee Online by March 2.
When the Affordable Care Act (ACA) was passed, with it came a host of reporting requirements for employers and health insurance companies. As of tax year 2015, KUSD is required to send benefit-eligible employees Form 1095C.
- You should receive your 2019 Form 1095C from Kenosha Unified by March 2, 2020, if you work 30 or more hours per week.
- You should receive your 2019 Form 1095B from the insurance carrier by March 2, 2020, if you were enrolled in health benefits AND work 30 hours or more per week.
While these documents are being provided for tax purposes, it is not needed to finalize or file your return. Benefit-eligible employees will simply be required to report whether or not they received benefits from KUSD in 2019.
