A smooth transition is our mission.

4 to 5 Years Prior to Graduation (8th & 9th Grade)

  • Begin student transition planner & participate in developing transition goals
  • Practice independent living skills (e.g.: shopping, housekeeping, budgeting etc.)
  • Begin graduation portfolio-include a resume & update it as needed
  • Attend career day
  • Participate in career exploration activities/classes
  • Identify learning styles & necessary accommodations
  • Learn to communicate interests, preferences & needs at IEP meeting
  • Visit a career center
  • Complete 4-year career plan
  • Identify vocational interests
  • Learn & practice informed decision-making skills
  • Pursue & use local transportation
  • Visit High Schools (8th grade)
  • Learn to monitor your own behavior

2 to 3 Years Prior to Graduation (10th & 11th Grades)

  • Continue transition planner & participate in developing transition goals
  • Participate in work experience/internship apprenticeship &/or enroll in Career, Technical & Service Education courses
  • Take PSAT test/ACT test & arrange for accommodations if necessary
  • Determine need for financial support
  • Visit Kenosha Job Center/Job Fair
  • Complete a vocational assessment inventory
  • Practice effective communication by developing interview skills, asking for help & identifying accommodations at post-secondary & work environments
  • Visit a career center
  • Investigate community agency connections for example: DVR, KAC, SAI, ADRC
  • Visit college campuses
  • Investigate job/class shadowing opportunities
  • Pursue guardianship issues before age 18
  • Add to graduation portfolio
  • Identify barriers that interfere with goals

Last Year Before Graduation (12th Grade)

  • Complete transition planner & participate in developing transition goals
  • Participate in work experience/internship apprenticeship/& or enroll in Career, Technical & Service Education courses
  • Take ACT/SAT (fall) & arrange for accommodations before registering
  • Request credit check for graduation from counselor
  • Complete graduation portfolio
  • Obtain paid employment
  • Complete college or technical school applications & arrange for accommodations
  • Apply for financial aid/scholarships
  • Identify community support services & programs. Complete applications procedures
  • Visit college campuses
  • Formulate final post-secondary plans & transition goals
  • Assume responsibility for health care needs
  • Register to vote & for selective services
