Grade 3 School Health Examinations
Our Wisconsin State Medical Society recommends that each school child have several health examinations during his/her school career. In accordance with this, the Kenosha Unified School District suggests that each child have a health examination before entering the fourth grade.
With this in mind, we urge you to take your child to your family doctor for a complete examination. We are all aware that good physical health is necessary for good academic progress.
Please ask your physician to fill out the enclosed health examination record form and see that this form is mailed to the school in the enclosed self-addressed envelope. The health record should be in your child’s file.
This information will be handled in a confidential manner, and the school’s will do all that we can to fulfill our responsibility in following any suggestions and recommendations, which the doctor makes.
